Oh man, you have NO IDEA how much this excites me! I was always so regretful that I made my paladin a female blood elf and not a male blood elf. But the idea of levelling another paladin made me feel ill. If I could just change that one thing about her I would be so happy!

No idea, as far as I know it's not even confirmed. But, given that name changes are currently $5 I wouldn't think it'd be much more.

I changed my mage's name. I originally rolled him on an RP server just to bork around with and so he had an RP name. When I realized I wanted to play him more full-time, I transferred him to my normal server and gave him a name that fit me better.
